Do you need to replace a card or fob?

If you have lost your access card or fob, please fill out this form:  I need to replace a card or fob.  Once we receive the form we'll send you an email letting you know when you can come to the fitness office to purchase the card or fob.  The cost is $25, payable by check, cash or card (note, if you pay by credit card there is an extra $1.50 service charge added on so you pay a total of $26.50).  There is a doorbell at the front entrance to ring when you come to replace the card or fob.
