Request for Modification Form

Please complete the following Architectural Control Committee (ACC) application if you are making any outside repairs or improvements to your home.  Prior to submitting this application - check the website for applicable guidelines, bulletins and required project details.

ATTENTION: Be aware that incomplete forms submitted without adequate project details may be rejected for insufficient information and will have to be re-submitted with more information. 

The review committee (composed of neighborhood volunteers) has up to 30 days to review your application (once all the project details have been provided); however, the average review time is usually much less (approximately 5-10 days). Once the review committee has processed your application you will be sent a written response regarding your request.

THIS APPLICATION WILL BE REVIEWED FOR THE LIMITED PURPOSE OF DETERMINING THE AESTHETIC COMPATIBILITY OF DESIGN WITH THE VARIOUS PROVISIONS OF WELLINGTON OF FLOWER MOUND HOA DECLARATION OF COVENANTS, CONDITIONS AND RESTRICTIONS. NO REVIEW HAS BEEN MADE WITH RESPECT TO FUNCTIONALITY, SAFETY, COMPLIANCE WITH GOVERNMENTAL REGULATION, OR OTHERWISE AND ANY PARTY WITH RESPECT TO ANY SUCH MATTERS SHOULD MAKE NO RELIANCE ON THIS APPROVAL. THE ABOVE SIGNED EXPRESSLY DISCLAIMS LIABILITY OF ANY KIND WITH RESPECT TO THIS REQUEST, THE REVIEW THEREOF, OR ANY STRUCTURES BUILD PURSUANT THERETO. THE ARCHITECTURAL CONTROL COMMITTEE MAKES NO WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, OR ANY OF ITS MEMBERS AND NONE ARE TO BE INFERRED FROM APPROVING OR DISAPPROVING THIS APPLICATION.
