Multi-Purpose Room Reservation Form

Maximum occupany of 100, with a maximum of 50 guests under the age of 18. There must be one adult chaperone in the room for every 10 guests under 18 years of age. Reservations can only be made by the hour (no 1/2 hour reservations allowed).
The room will be available to you at START TIME and you must be cleaned up and out by END TIME.

For parties using the pool, only 20 guests may be at the pool at any given time. There must be one adult chaperone in the pool area for every 3 guests under 18 years of age in the pool. SWIM AT YOUR OWN RISK, LIFEGUARDS ARE NOT PROVIDED.

For catered parties, the caterer must retrieve all supplies, materials and unused food/drink by the end of the event. Failure to remove items will result in a $25 per hour charge to the resident hosting the event until such time as the items are removed from the premises.

Make all checks payable to Wellington of Flower Mound. Cash not accepted. Check must be from the resident's account. No third party checks accepted. Payment must be received within 5 days from submission of request or the reservation will automatically be cancelled. Cancellations received 14 full days prior to the scheduled event will receive a full refund (excludes re-occuring events). Re-occurring events may only be booked for a 5 month period and are limited to 11 separate dates. Re-occurring events may not reserve the MPR: Fri 5:00pm - Sun 10:00pm.
Failure to comply with all policies of the HOA (including but not limited to the following) could result in assessment of fees or suspension of reservation privileges.
(1) Balloons which lodge on/in the ceiling, fans or light fixtures result in a fine starting at $25.
(2) Falsely setting off the fire or security alarm results in a $50 fine.
(3) Applicant must be present at the event 100% of the time and cannot reserve the room for a non-Wellington individual.
(4) Applicant may NOT arrive early or overstay their reserved time. Failure to comply will result in additional charges. One event may not be broken into two separate reservations to reduce fees.
(5) Applicant must clean the room after their event, including but not limited to; vacuuming (vacuum not provided,) table/chair stacking, trash removal, (dumpster located in parking lot) etc. Failure to clean properly will result in a cleaning fee starting at $30.
(6) Lingering odors from food or trash, such as fish, garlic, curry, onions, broccoli or eggs will result in a $25 fine for odor removal.
(7) Applicant is financially responsible for any damage which results from their event and guests.
(8) Applicant and guests must adhere to all instructions from any Wellington staff.
(9) Doors to the clubhouse may NOT be kept open. Failure to comply will result in a $25 fine. Applicant must assign someone to monitor access.
(10) Alcohol and tobacco products are prohibited on HOA premises. Failure to comply will result in the immediate cancellation of the event.
(11) The clubhouse and pool close at 10:00 pm and everyone must leave the facilities by that time. Failure to comply will result in a $25 fine for every 15 minutes over the 10:00 pm closing time.
(12) A processed check returned for any reason (e.g. NSF) will be assessed a service fee of $25.
(13) Access to the Fitness Center, entrance hall, or locker rooms may not be restricted from Wellington residents.
(14) The entrance foyer and conference room is not part of the reservation and all activities must be kept in the Multi-Purpose Room.
(15) Wellington Home Owner Association events and meetings take precedence over private events and as a result there is a minimal risk my reservation may be cancelled.
